Fertility Treatment Costs and Plans


In recognition of the difficult economic times, we at Jacksonville Center for Reproductive Medicine have developed new pricing plans to help our patients who have no IVF coverage in their insurance plans. A list of accepted insurances for fertility treatments, including IVF and Tubal Reversal, is listed here . We hope that these plans will assist in reducing  some of the financial burden and stress that the IVF process may impose. JCRM is happy to honor our military patients by offering a 10% military discount (valid on services considered not eligible for reimbursement by Tricare)

This program has been instituted in response to the recession in our current economy and is for our self-pay patients who do not have insurance coverage for IVF services.

Four Cycle IVF Lower Cost Plan Top

To maximize IVF success, up to four (4) cycles of IVF are offered to most couples at a reduced cost. For patients who would like to purchase an IVF treatment plan at a reduced, fixed cost, we are now offering the Four Cycle IVF Lower Cost Plan.

What is included?

  • Four fresh cycles and all frozen embryo transfer cycles within an 18 month period after the commencement of the initial cycle
  • Monitoring ultrasounds and bloodwork
  • Egg retrieval, anesthesia and facility fee
  • Embryology laboratory services including ICSI and/or assisted hatching (if indicated)
  • Embryo cryopreservation and storage for one year after incident fresh cycle

What is excluded?

  • Diagnostic testing
  • ALL MEDICATIONS
  • Outside monitoring (another facility)
  • PGD
  • Donor services (sperm and/or egg)
  • Pregnancy monitoring and labwork

There is no initial entry criteria for this plan, other than no health insurance coverage for IVF services. If the patient achieves a pregnancy which results in a live birth with a child taken home from the hospital prior to completing their fourth fresh cycle, the plan will be deemed complete. Frozen embryos must be used prior to initiating another fresh cycle. This is not a refund plan.

The cost for this plan is $17,850. Payment is due in full two (2) weeks prior to baseline ultrasound.

If, for medical reasons (usually due to poor stimulation/prognosis), the patient and physician mutually decide to terminate the Plan prior to the third fresh IVF cycle, charges will be computed as per the Traditional IVF Plan. Any credit balance remaining will be applied to the next phase of treatment which would often involve IVF utilizing an egg donor.


 
IVF Refund Plan Agreement Top
 
 
Many couples seek more guarantees and a set initial cost for their IVF treatment. In this plan five (5) fresh cycles are offered at a reduced, fixed cost. If the plan does not result in a live birth with a child taken home from the hospital, 70% of the cost will be refunded.
 
Who qualifies?
  • Patients with no health insurance coverage for IVF services
  • Age
  • Normal ovarian reserve screening, no history of endometriosis
  • No more than two prior IVF cycles at outside centers
  • Stimulated endometrial thickness of ≥7mm
  • No hydrosalpinx or significant fibroids unless corrected prior to cycle start
  • Semen analysis not indicative of azoospermia (no sperm) or severe oligospermia (very low count)
  • Post-vasectomy patients do qualify, but will need a sperm retrieval (at an additional cost)
Criteria will be evaluated on a case-by-case basis. Some patients will not automatically be excluded if they fall outside the above criteria.

Egg Donor IVF Refund Plan

Many couples seek more guarantees and a set initial cost for their IVF treatment. In this plan four (4) fresh cycles with use of egg donor are offered at a reduced, fixed cost. If the plan does not result in a live birth with a child taken home from the hospital, 100% of the cost will be refunded.
 
Who qualifies?
  • Patients with no health insurance coverage for IVF services
  • Age ≤50; BMI <35
  • Normal ovarian reserve screening, no history of endometriosis
  • Have not failed more than one prior egg donor IVF cycle previously
  • Stimulated endometrial thickness of ≥7mm
  • No hydrosalpinx or significant fibroids unless corrected prior to cycle start
  • Semen analysis not indicative of azoospermia (no sperm) or severe oligospermia (very low count)
  • Post-vasectomy patients do qualify, but will need a sperm retrieval (at an additional cost)
Criteria will be evaluated on a case-by-case basis. Some patients will not automatically be excluded if they fall outside the above criteria.

  • Basic donor evaluation

  • Recipient requested genetic testing of donor
 
The cost for this plan is $35,000. Payment is due in full two (2) weeks prior to baseline ultrasound.

Tubal Reversal

There are very few insurance policies that cover this procedure. If you do have insurance coverage, it will be your responsibility to obtain any necessary pre-certifications and/or authorizations.

WINFertility Program

WINFertility PROGRAM INFORMATION The WINFertility Quality Discount Treatment Program is offered exclusively in the Northern Florida area to JCRM patients who have no infertility insurance coverage or who have exhausted their benefits and seek an efficient, economical way to obtain and pay for high quality advanced fertility treatment. The WINFertility Program includes both advanced medical treatment and fertility medications in a single, discounted ‘bundled’ price for each cycle. Treatment Bundle Prices for IUI, IVF and Third Party Reproduction are 10% to 20% less than fee-for service medical fees and retail pharmacy rates in the Greater Jacksonville marketplace. There are no clinical or age requirements to participate. Financing programs are available to make affording treatment even more manageable. For more information contact:
